In the 5+ years of reselling and buying lots, I've come across a 3 or 4 different "homebrew" character sheets. Some pretty nicely done - even attached at the top (like a notepad).
My guess is they are just that . . . a homebrew (or game store brew) set of character sheets. _________________ "Ah, Blackadder. Started talking to yourself, I see."

Is that about all the print? I have noticed a few fistful of these anonymous types of things (not precisely the same) with some of the mapping aids for gaming. _________________

Regarding the character sheets above, I've been going through my collection, cataloging everything. I came across a set of AD&D character sheets that came with Of Skulls and Scrapfaggot Green by Bob Blake. There is a Generic, a Paladin/Ranger, a Psionicist, a Wizard, and a Warrior/Rogue. There is also a sheet that is meant to serve as a back to the others. I couldn't tell you if these sheets were used for a tournament, but it does suggest the possibility that tourneys had their own sheets.

I have seen a couple very professional homebrew pads over the year, but not that one. I have the name of one of those homebrew companies at home that have one that looks similar. I will look it up tonight.

The closest one I have is from a company (and I use that term loosely) called The Masters Lab. It is very similar. I have scan and I will post it in the next couple days when I can find it again. _________________ pacesettergames.com
